  • Copy of current and Valid Driver’s License.

  • Must provide a copy of your Motor Vehicle Record, certified from DMV. (Printed or PDF)
  • 5 year FBI Background Check and Finger Print Qualification Letter. (Can be performed at the Breckenridge, Frisco, or Silverthorne Police Station)
  • Valid/Current CDOT Physical & Medical Examiner’s Certificate.
